For all Applications:

Must be a resident of Summit County.

Must be at least 25 years of age (maybe younger if there are dependent children in the household.

Copy of driving license front and back (showing current address)               

Proof of residency 6 months or more in Summit County (Rental agreement)

Copy of current bank balance. (If you have account in your name)

Copy of pay stubs of for last 30 days (if employed) First two pages of tax return, if (self-employed).  or proof of other income.

All applications are expected to attend a mandatory car care class provided by Miracles on Wheels, held on Saturday in April or October.

Must be drug free and have no extensive criminal background.

Must have no current or pending criminal or DUI/ DWI charges.

Must be insurable.

Must have income in the moderate – income range

For car Donation request: please note for car donation, after all the information is received, Verify. and accepted, you be put on a waiting list. Once a car is available, you’ll be notified. Once you’re notified, you’ll be responsible for obtaining car insurance.

Application with incomplete information or documentation. After 30 days you will be removed from the waiting list.

Submit your complete application in additional document by mail only. We will mail out an application to you. Once you contact us.
